individual/family membership fee covers the cost of insurance for you
and now for your dependents as well! Please be sure to provide us
your name, address, phone number, email address, date of birth.
Also if you are adding dependents, provide their names, dates of birth
and relationship to you. You can give this information to us
by adding "special instructions to the seller" just under your address
on your order,

($2 online convenience fee included
for online payments)
If you are already a member of another MSA Club, you can
join our club for $10.
